Navigate to [Tally](https://try.tally.so/100days)

Click "**Sign up**" and follow the sign-up instructions.

Once you've finished signing up, click "**Create form**" within the dashboard.

Give your form a name by clicking the form title field and entering your text.\

If you're following along with this example, we're going to create a feedback form for students currently enrolled in one of the 100 School challenges.

Click on this text or press enter to start your form from scratch.

While we're here, we might as well add a little customisation to the form by adding our logo. \
\
Click "**Add logo**"

Click "**Click to choose a file or drag here**" and upload a logo from your device.

If successful you'll see your logo added to the top of the page.

The first thing we're going to do is add a question with conditional logic to ensure that we only receive responses from students that are currently enrolled in #100DaysofNoCode or #100DaysofAI.\

Click "**Insert block below**"

Select "**Multiple choice**" within the "Questions" section.

**Add your first question.** \

In this example we're going to ask users whether they are a student currently enrolled in one of the 100 School challenges. They'll then be served different outcomes based on their response.\
\
Click inside of "**Option 1**" and add "**Yes**"

Click on a new line to create a new block. We're now going to add some text that will only appear when users select "No".

Enter the text you'd like to be shown to users that are not qualified to complete the feedback form.

This feedback form is only for students that are currently enrolled in one of our challenges. If you havea query or feedback you'd like to give outside of our challenges, please email 

Click the icon highlighted below and select "**Hide**". \

What you're doing here is making sure that this block is hidden within the form, until it's triggered to show by user response.

Select the option "**No**" as the conditional logic trigger within the "**When**" section.

In the "Then" section,  click "**Hide button to disable completion**"\
\
This removes the ability for users to submit the form, which is exactly what we want because we only want to collect data from currently enrolled students.

Click this icon in the "**Then**" section and select "**Add action**"

Select the text block that you want to show to users that aren't qualified to take the survey.

Great work, you've already learned how to use conditional logic within a form.\
\
In this example, users that select "No" won't be able to submit the form and will be shown a block of text explaining why.\
\
Now we'll continue to build the rest of the form.\
\
Click this icon (highlighted below) to insert a new block.

Select "**New page**" within the "Layout blocks" section.\
\
What you're doing here is separating your first qualifying question from the rest of the form.

You should see a page divider block appear as shown below.\
\
Next, click the "**Insert block below**" icon.\
\
You'll now add survey questions for users that *are* qualified to take the survey.

Select "**Multiple Choice**" within the "Questions" section.

Insert your question and multiple-choice options.\
\
In this example, we're going to ask students if they've been able to complete most lessons in 30 minutes or less, giving them a "Yes" or "No" response option.

After you've completed the first question, click a new line and click the "**Insert block below**" button to add your next question.

Select "**Long Answer**" within the "**Questions**" section.

Here we'll ask an open-ended question to collect feedback on user's favourite lessons, using the long-text response format.

Once you're happy with your long-text question, click a new line and click the "**Insert block below**" button.

Select "**Rating**" within the "Questions" section.

For this question, we'll ask users to rate their current experience of the challenge they're enrolled in on a scale of 1 to 5.

Once you're happy with your rating question, click "**Insert block below**"

Select "**Long Answer**" within the "Questions" section.

We'll now ask a question that allows students to detail any aspect of the challenge that has fallen below their expectations, using the long-text format response.

Finally, we'll add another long-text block, allowing users to submit any additional feedback they'd like to give on their challenge.\

Click "**Insert block below**"

Select "**Long answer**" in the "Questions" section.

Enter some text that encourages users to leave additional feedback.

Okay, that's our form questions all ready.\
\
Let's just tweak the design of our form slightly so that it's consistent with our brand.\
\
Click "**Design**" at the top of the form.

Click "**Preview**" and your form will open up in preview mode for you to interact with.\
\
Check everything is working as expected (pay special attention to the conditional logic that you configured earlier).

If you're happy that everything is working, click "**Back to editor**"
