Click the "First Name" field.

Put in the first and last name

Click the "Title" field and put their title.

Click the "Phone" field.

Type in the phone number.

Click the "Email" field.

Click the "Website" field.

Click the "Name of the referrer" field.

Type your name or who you received the lead from.

Click the "*Brand Lookup" field.

Type the brand.

Switch the brand name here also.

Type in the company name.

Edit the lead source.
